Church Lab Ordering Form is no longer used for orders

Please fill out the following order form, one per company.

By request, all Church lab orders from 5/99-7/00 can now be viewed online for cutting and pasting of old ordering information. The tables are large (>250k) so will take a few moments to load. Sorted by:
- Date
- Vendor
You can now access the Genetics Office database to look up previous orders (from 5/99), check on the status of an order, or look up a PO number (e.g., Genosys, ATCC, Genome System). Click here for instructions.
All orders sent before 1:00 pm will be called in by Eva on the same day. Orders after 1:00 pm will go into the purchase requisition of the following day. Most orders arrive within a week or so.
Please add any additional comments (e.g., rush requests) in the notes box at the bottom of the form.
